Between-Visit Care (PAWS)
Sigmund's clinical assistant handles the visit. The research program's next phase extends care into the weeks between appointments, closing the loop from encounter to ecological monitoring to the next clinical decision.
PAWS wearable
PAWS is a wrist-worn companion built on the Google Pixel Watch, pairing passive sensing with an LLM-driven therapeutic agent. The program is advancing through an NIDA UG3/UH3 award toward clearance as a software-based medical device, developed in collaboration with Google.
The between-visit monitoring loop
Care does not stop when the visit ends. PAWS reads ecological signals continuously: sleep, activity, circadian and behavioral rhythm, physiological arousal, and adherence the patient could never accurately recall in clinic. That signal surfaces at the next decision, so the clinician adjusts with data, not recall alone.
Condition-specific research directions
Depression
Behavioral activation, re-engaging the patient with rewarding, values-aligned activity, delivered and tracked between visits. The clinician titrates against real behavior instead of self-report colored by the depression itself.
Anxiety
CBT skills and mindfulness practice extended through the companion, with engagement and physiological response tracked between sessions, so the clinician knows what was practiced and what worked.
Schizophrenia
Clozapine documentation support to keep the most effective antipsychotic in place, plus early-relapse signal monitoring (sleep, activity, social rhythm) while there is still time to act.
PAWS is investigational and not yet available in Sigmund. It represents the Sultan Lab's next research phase, building on NIH grant infrastructure and the CEBA evidence-engine program.
